Questions regarding progressiv overload

I've been training for a lot of years now but not really seeing any major results. So i tried to constantly rethink the things i do and try to erase mistakes. Now im onto overload.
If i e.g bench 70kg 12x 10x 8x in week one - and try to go for more but really cant manage to get even 1 more rep in - is it okay to do another set to failure. And try next weak to get more reps in the 3 sets in? Or do i increase the weight and do less reps? Or do i just try to get more reps in until it somehow works? Or whatever?
I think i really avoided the topic how to go about progressiv overload, because i was lazy and hoped results would come without thinking about that too much. Any advice is greatly appreciated.
