Tips for coming back from the dead

So I was laid off last October from a very labor intensive position and will be returning shortly, in the meantime I've spend the time I've had off passing out at 5am and waking up at 2 in the afternoon just to jump into some games and maybe eat takeout once or twice if I didn't have enough easy stuff in the house.

So my question is where do I start and what areas should I focus on to go from not sleeping eating or moving and vaping heavily for 5 months straight to swinging a sledge hammer for 12 hours a day 7 days in a row without absolutely embarrassing/hurting myself

All advice welcome, anything i read here will be more work than I've done toward getting back on track so far
